Team check-in will take place Saturday August 18th on the North side of Tramore Park near the concession stand.
Please arrive 45 minutes before your teams first scheduled game with the following:
- 2 copies of the roster release form
- Player passes or birth certificates for each player and guest player
- A medical release form for each player and guest player
- A copy of your state roster or one generated by your club administrator
- 4 game cards with rosters completed
2. Player Passes
Player passes will be checked against the Player
Roster/Release Form.
3. A Copy of your Current State Roster
We will keep the copy. Add information on any guest players
to this copy. Please have the original available in case of
discrepancies.
4. Medical Release
Forms (waiver)
Medical Release forms will be checked against the Player
Roster/Release Form. There must be a medical release form
for each player listed.
5. Official roster

Each form makes rosters for two games. You will need a total
of four (4) at check in. It is in PDF format. If you type
your team information in the top part of the page, it will
automatically copy it to the lower part so you will not have
to fill it out twice. Download the form by clicking
here. If it does
not download directly to your computer you may Right Click
on the form to download it, or click and drag it to your
desktop.
